Camp and Cozumel Physical Forms - PLEASE PICK THE CORRECT FORM
|
|
NOTE: there are two different health forms. Please see the descriptions below to help choose the correct form. Summary - If you are participating in any high adventure activity OR are over 40, choose the Class 3 Physical. Otherwise choose the Class 1 and 2 Physical. If you already have a Class 3 physical, it will work for all activities.

|
IMPORTANT - please take time to complete all of the information on the physical forms carefully. There are important details such as emergency numbers, insurance carrier information, medications and allergies that must be available in an emergency. Please check the information for completeness and accuracy!

BSA Class 1 and 2 Physical -->
|
|
<- Use for Scouts - normal activities. Complete Class 1 AND Class 2. Health exam must be within 36 months prior to camp. |

BSA Class 3 Physical (adults >40 and High Adventure including Climbing, COPE, Caving , Mountain Biking) ->

<- Use for Adults over 40, Scouts in Medicine Mtn. High Adventures and Cozumel Scuba. Health exam within the past 12 months required. |
