MEDICAL FORMS, CAMPING AND SAFETY LINKS

| ALL BSA MEDICAL FORMS NOW REQUIRE ANNUAL MEDICAL EXAMS AND DOCTOR SIGNATURES.
There are 3 different medical forms required by BSA depending on the location and duration of the trip. Colorado state law requires a different form than other states. The national form does not work for Colorado camps such as Ben Delatour. NOTE - the BSA National Form is an interactive pdf form. It can be completed on the computer, saved and printed for future use. IMPORTANT TO MAKE COPIES - Most summer camps keep the medical form. Please make copies before submitting. We strongly suggest that you NOT include Social Security Numbers. We have never needed it at any emergency room in Colorado, Utah, Wyoming, Alaska or South Dakota. If we need it, we can get it by phone. NEATNESS and COMPLETENESS - our biggest problem in emergency situations has been the completeness of the forms. Please take extra time to make sure that every line, box and signature is completed. Please check all forms carefully. Many forms require both parents (if applicable) and Scout signatures. |

BSA now requires that all registered adults complete Youth Protection Training (YPT) every two years. New applicants MUST complete YPT before registering AND include a copy of the completion certificate with their application.
